Project Overview
Apollo Urgent Care Check-in app was part of a group project in my Human-Computer Interaction class. The instructions were to create a check-in form for an urgent care center that the incoming patients would fill up when the enter the urgent care location to get a doctor assigned to them. We had to employ everything that we had learnt in class till then with the objective of creating a check-in form that patients could fill in as soon as possible.
My Contributions
As this was a group project/assignment to be completed in just 2 days, the primary objective was to have a prototype. My contributions mainly included deciding on the flow of screens, and deciding on how a patient can get assigned a doctor as quickly as possible. I also helped in designing the prototype.  
Wireframe
The wireframe depicts the user's journey and shows how the screens will look and the order. This gave the team a guideline on how to design the patient check-in form.
Screen 1
A basic welcome screen to be displayed to all the patients. The color blue as the background is chosen because it depicts credibility, trust, knowledge, power, professionalism, cleanliness, calm, and focus.
The instruction on this screen is clear, the patient needs to touch the screen to get started.
Screen 2
This screen is mainly for people who have an emergency and cannot wait for a doctor to be assigned to them. If a patient clicks on 'Yes', a doctor is immediately assigned to them without filling out any form.
Screen 3
A doctor is immediately assigned to an emergency patient. There is no wait-time for an emergency patient.
Screen 4
In Screen 2, if the patient selects the 'No' option, the patient is then asked if they are a returning patient. This is done so that returning patients can easily access their existing patient records.
This makes it easier for returning patients to access their records and get a doctor assigned to them. It is observed that people generally like to go to the same hospital/urgent care.
Screen 5
When the patient is a returning patient, they get the option to schedule an appointment using their insurance number, insurance provider, and date of birth.
To make things easier, they can scan their insurance card to get the details immediately without typing anything.
Screen 6
In Screen 4, if the patient selects the 'No' option, the patient has to then fill the check-in form. The form asks for all the relevant data.
Screen 7
Once the patient completes their check0in form, a doctor is assigned to them. The estimated wait is shown so that the patient has a clear idea about how long they have to wait.
